COVID-19 Vaccination & Negative Test Requirements

Sugar Swing requires proof of vaccination or negative rapid test results to participate in our dance socials and classes. This means we will be:

  • Requiring proof of vaccination (double dose) or negative rapid test results upon entry to all in-person services with Sugar Swing Ballroom
  • Verifying identity with a driver’s licence, passport, or health card

For the proof of negative COVID-19 test results:

  • Proof of negative PCR or rapid test within 72 hours of service; privately-paid, AHS, or Alberta Precision Labs

Exemptions to be made:

  • Medical reasons – will need to provide documentation of a medical exemption
  • Children under 12 – will be permitted entry accompanied by an adult, who must be vaccinated or have proof of negative COVID-19 test result per above

If an individual is unable to provide sufficient proof of vaccination, negative test results, or medical exemption, they will be refused entry. However, Sugar Swing Ballroom may be able to provide accommodation upon request to some of our services. Accommodation could include online participation at our dance socials or classes.

QR Code (Alberta Vaccine Record)

Obtain your QR Code through Alberta.ca (https://covidrecords.alberta.ca/home)

Online (through MyHealth Records)

  • Albertans 14 and older can view their verified health records, including all vaccines, lab tests and medications, through MyHealth Records, an online tool that lets you access your personal health information from any computer, tablet or smartphone. To sign up, follow the instructions on the website.
  • MyHealth Records will be automatically updated up to 2 to 3 weeks after your dose if you were vaccinated in Alberta. From your account, you can:
    • print a convenient size card (starting September 19)
    • download and use the app to show or take a screenshot of your record

Paper Records

  • You will receive an immunization record at your vaccination appointment.
  • If you lost or require another paper copy, you can print one from MyHealth Records or request a copy through your immunization provider:
    • Pharmacy or physician’s office – contact the location you received your vaccination
    • AHS Clinic – call 811 to request a copy of your record mailed to you
  • Albertans between the ages of 12 and 14 must obtain a paper record from their immunization provider.

***For the purposes of security and confidentiality, no physical or digital record of the proof of vaccine, proof of identification, or any medical documentation will be kept by Sugar Swing.***
